** The link below will take you to the UW Madison donor screening form. Here you will fill in personal information about yourself. A few notes on the screening form:

1) On the "Donation details" screen, my information is in the screenshot below. I filled out the page as though I was an unrelated donor (i.e. you) to help clarify any confusing questions.

2) On the "Voucher program" screen, you'll want to click "I want to donate as a directed donor" so that the kidney is matched to me.
